4. Upon receiving a notice of reference, the registrar shall enter particulars thereof in the Register of References, and shall forthwith send a copy of the notice to each of the parties to the proceedings (other than the party or parties by whom the notice of reference is signed), together with a notice substantially in accordance with Form 2 in Appendix A, and shall inform all the parties of the number of the reference. The name of the Tribunal, the names of the parties and the number of the reference, together with the year in which is instituted, shall thereafter constitute the title of the proceedings.
